1. Public participation  
 A. Before the recommended screening criteria for control alternatives were developed, was a letter sent to the following describing the CSO issues and inviting their participation?
  • All neighborhood associations in combined and sanitary sewer service areas
  • All sources with wastewater permits from the municipality or IDEM that discharge to combined and sanitary sewer systems.
  • Known local conservation and environmental groups in county and first county downstream of discharge.

 B. Were people who attended the public participation events put on a mailing list and regularly sent updates on the issues and events (unless they asked not to be on mailing list? 2
 C. Does the summary of the public participation plan include:
  • Date, time, location of each event and number of participants; and
  • Summary of comments received and changes or decisions made in response to public comments?

 D. Did advisory group include people who:
  • Live in the neighborhoods where discharges occur; and
  • Regularly use the waters downstream of the CSOs?

 E. Were draft and final versions of LTCP provided to all public libraries in combined and sanitary sewer service areas? 2
2. CSS Characterization, Monitoring and Modeling  
 A.  Does LTCP include a map that clearly indicates:
  • CSO outfalls;
  • Receiving waters;
  • Areas served by each CSS interceptor;
  • Sensitive areas including parks and common access points; and
  • Areas served by each sanitary sewer interceptor that flows into a CSS interceptor.

 B. Does LTCP include a map or description that indicates median income and minority population of each census tract along the receiving waters where the CSOs occur as well as two miles upstream and downstream?* 2
 C. For sources with wastewater permits from the municipality or IDEM that discharge into combined or sanitary sewer system, does LTCP include:
  • Contribution of each source to the volume during dry weather;
  • Contribution of each source to the volume during a 1-year and 5-year flow; and
  • Pollutants from each source that are discharged on a monthly average at levels above the NPDES permit limit for the primary system discharge point (typically 001)?

3. CSO Control Alternatives  
 A. Does a LTCP alternative require that new sanitary sewer connections are offset by reductions of other dry weather flows? 3
 B. For sources with wastewater permits from the municipality or IDEM, does a LTCP alternative include:
  • Enforceable requirement that wastewater and/or storm water discharge be held back when a CSO is anticipated; and
  • Quantity of wastewater and/or storm water that is expected to be held back during typical operations?

 C. For existing roof drains in CSS area, does a LTCP alternative include requirement that they be disconnected as part of any sewer repair and that owners be billed for estimated flow from roof drains? 1
 D. Does LTCP consider mix of four general categories of controls (plus real-time controls for communities with more than 50,000 people) that are designed to capture a 1-year rainfall event? 2
 E. Does LTCP identify public health and environmental benefits that would result from implementation of each CSO control alternative? 3
4. Selected CSO Controls  
 A. Is ultraviolet light disinfection used for CSOs during recreational season? 2
 B. Will CSOs under LTCP to sensitive areas be reduced or eliminated? 3
 C. Will CSOs under LTCP achieve State WQS within five years for:
  • Aquatic protection;
  • Recreational use protection; and
  • Drinking water protection?

 D. If State WQS will not be met, will sewer rates exceed $30 for a typical household with 5000 gallons of monthly water usage? 2
 E. Does LTCP indicate whether WQS would be achieved if sources other than combined sewer system were excluded? 1
 F. Is LTCP consistent with watershed planning efforts and TMDL (if one applies) 2
 G. Will the residents and stream users be notified of anticipated CSOs when they occur? 3
5. Implementation Schedule  
 A. Does the LTCP put sensitive areas and removing potential disproportionate impacts on minorities as top priorities? 3
 B. Does the schedule include milestones with clear deliverables for major implementation activities? 1
 C. Does the monitoring program include measures of success that will be reported to IDEM and local conservation and environmental groups at least quarterly. 3
