• You can nominate yourself, your organization, another person, or another organization.
• You may nominate more than one person or organization.
• Nominations must be received at 5244 Carrollton Ave, Indianapolis, IN 46220 by October 9 at 5:00 pm.
• Nominations may be submitted by email or by fax. Please provide a hard copy by mail as soon as possible.
• The Task Force may give more than one award or may give no awards in a particular category.
